Key Responsibilities
- Provide direct and indirect patient care following the nursing process of Assessment, Planning, Intervention, and Evaluation.
- Develop and update nursing care plans in coordination with patients, families, and interdisciplinary team members.
- Communicate changes in patient’s clinical condition to physicians, supervisors, and team members.
- Assist with discharge planning and ensure smooth transition of care.
- Perform patient assessments upon admission and during reassessments, documenting thoroughly in medical records.
- Administer medications accurately, following the five rights of medication administration.
- Assist physicians with examinations, treatments, and procedures within scope of practice.
- Educate patients and families on care plans, pain management, and health education, addressing psychosocial and cultural needs.
- Maintain high standards of nursing care according to hospital and departmental policies.
- Supervise and evaluate nursing team members, assigning care based on patient needs and team member capabilities.
- Serve as a liaison between administration, healthcare providers, patients, and families.
Required Qualifications
- Current state licensure as a Registered Nurse.
- BCLS certification.
- Minimum of six months’ Medical/Surgical experience in an acute care setting preferred.
Preferred Experience / Skills
- ACLS certification preferred.
- Experience working in a Long-Term Acute Care (LTAC) setting is a plus.
- Strong communication, organizational, and interpersonal skills.
- Basic computer skills for documentation and communication.
