Contract | Full-Time | Days | Monday–Friday (flexible scheduling options available, including shifted schedules)

Key Responsibilities

• Assess clients' needs and develop individualized therapy plans; administer and perform therapeutic interventions across SNF and Home Health settings

• Document observations, services provided, and client progress accurately within electronic health record systems

• Notify physician of changes in client condition and process orders; provide direct consultation with physicians and interdisciplinary team members

• Provide training on therapeutic programs, adaptive equipment, and other interventions to clients and care team members

• Supervise therapy assistants in accordance with applicable Federal and State regulations, documenting supervision appropriately in the clinical record

• Maintain client self-esteem, privacy, and confidentiality while promoting independence and choice consistent with individual care plans

Required Qualifications

• Completion of an accredited physical therapy program required

• Current national registration and state licensure for the state of practice

• Must be at least 18 years of age

• Ability to communicate effectively in English, both orally and in writing

• Demonstrated ability to supervise therapy assistants in compliance with regulatory standards

Preferred Experience / Skills

• Prior work experience as a therapist in a SNF or Home Health clinical setting

• Familiarity with Net Health and/or HCHB documentation software

• Experience working across multiple care settings, including skilled nursing and home health environments
