• Full-time, Days shift (8 hours per day)

Key Responsibilities
• Provide skilled physical therapy services to home health patients in accordance with physician orders and treatment plans.
• Perform patient assessments and develop individualized therapy plans.
• Document patient progress using EPIC/OASIS systems accurately and timely.
• Collaborate with healthcare team members to ensure quality patient care.
• Travel within the service area, optimizing routes to minimize drive time.

Required Qualifications
• Valid Physical Therapist license for Oregon or pending license approval.
• Minimum of 1 year of skilled home health experience.
• BCLS certification.
• Own reliable transportation.
• COVID-19 vaccination including primary series and one booster.

Preferred Experience / Skills
• Experience using EPIC and OASIS documentation systems.
• Ability to manage a caseload of up to six patients per day with a 1:1 ratio.
• Knowledge of home health regulations and documentation standards.
