Key Responsibilities

• Provide outpatient physical therapy services within designated counties including San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, and Los Gatos.
• Travel outside the primary service area may be required to complete patient visits.
• Conduct assessments, develop treatment plans, and deliver therapy sessions to a diverse patient population.
• Complete all documentation accurately and timely, including OASIS assessments and related reports.
• Ensure compliance with all safety and health protocols, including caring for COVID+ and COVID Pui patients when necessary.
• Maintain professional credentials and stay current with mandated licensure and certifications.

Required Qualifications

• Valid California PT license.
• Current BLS certification.
• Valid driver’s license, auto insurance, and vehicle registration.
• Proof of TB test, MMR, Varicella, HepB, Tdap (within 10 years), and annual flu vaccination.
• Successful completion of drug screening and physical assessment.

Preferred Experience / Skills

• Experience working in outpatient therapy settings.
• Ability to travel and adapt to changing patient locations and schedules.
• Knowledge of OASIS documentation and outpatient therapy protocols.
