Key Responsibilities
- Assess patient and caregiver needs using the nursing process to provide high-quality care in compliance with regulations and policies.
- Develop and communicate individualized care plans, ensuring patient and caregiver inclusion.
- Serve as Case Manager, actively managing caseloads and visit times to meet productivity standards.
- Complete accurate OASIS documentation in alignment with agency policies and federal/state requirements, staying current with documentation updates.
- Implement and oversee medical plans of care, nursing interventions, and procedures to ensure patient safety and care quality.
- Maintain proficiency in high-tech skills such as IV therapy, wound VAC, and complex wound care, adhering to infection control standards.
- Collaborate with LPNs to ensure compliance with the care plan, delegate tasks appropriately, and review documentation regularly.
- Evaluate care effectiveness by reviewing patient responses and adjusting the plan as needed to meet patient needs.
- Document nursing care accurately and timely to support ongoing quality care and inform the healthcare team.
- Follow agency protocols, including proper use of medication management and visit documentation systems.
- Provide education to patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions.
- Identify interdisciplinary needs and communicate with other healthcare professionals efficiently.
- Complete and submit required agency, state, and federal forms accurately and promptly.
- Conduct visits efficiently, coordinating with schedulers to avoid combining supervisory and routine visits.
- Assess patient needs for home health services, develop and coordinate care plans, and supervise ongoing care.
- Report incidents such as infections or injuries to the QI Specialist in a timely manner.
- Manage discharges and transfers in accordance with policies and regulations, ensuring proper documentation.
- Collaborate with physicians, providers, and team members to coordinate patient care smoothly and safely.
- Participate in peer review audits and case conferences, contributing to continuous quality improvement.
- Advocate for patient care needs, ensuring sensitive and personalized care delivery.
- Conduct comprehensive assessments during admission and regularly update the care plan with the interdisciplinary team.
- Stay current with pain management, symptom control, and high-tech nursing skills.
- Educate patients and caregivers on treatment options, medication management, and discharge planning.
- Coordinate with healthcare teams, vendors, and providers to ensure safe and effective patient care.
Required Qualifications
- Valid RN license in the state of Vermont.
- Experience with OASIS documentation.
- Knowledge of federal and state healthcare regulations, including Vermont State Board of Nursing standards.
- Proficiency in high-tech nursing skills such as IV therapy and wound care.
- Strong documentation and communication skills.
- Ability to collaborate effectively with interdisciplinary teams.
Preferred Experience / Skills
- Previous home health nursing experience.
- Experience with case management.
- Familiarity with McKesson documentation processes.
- Bilingual abilities are a plus.
