Schedule / Job Type

• Full-time, Day shifts (5x8 hours)

Key Responsibilities

• Provide skilled outpatient physical therapy services to home health patients in West Portland and surrounding areas
• Assess patient needs and develop personalized treatment plans
• Travel to up to 6 patients per day within a maximum radius of approximately 60 miles
• Document patient progress using electronic health record systems such as EPIC and Oasis
• Collaborate with care teams to ensure optimal patient outcomes and compliance with facility protocols
• Maintain professional standards and adhere to safety and transportation requirements

Required Qualifications

• Valid Oregon State Physical Therapist license
• Minimum of 1 year of hands-on skilled home health experience
• BCLS certification
• Reliable personal vehicle for travel to patient homes
• Ability to work daytime hours, 8 hours per shift, Monday through Friday

Preferred Experience / Skills

• Experience with Oasis documentation system
• Strong organizational skills and ability to manage a patient caseload efficiently
• Familiarity with EPIC or similar charting platforms
