Community Hospice - Case Manager (Registered Nurse)

Schedule / Job Type
Contract | Days | Monday–Friday, 8am–5pm with occasional weekend back-up and on-call
Key Responsibilities
- Manage an assigned caseload of hospice patients, leading the interdisciplinary team in delivering personalized care
- Provide compassionate clinical care to patients wherever they call home, including private residences, nursing homes, group homes, and assisted living facilities
- Educate patients and caregivers to help identify personal goals of care and build caregiver confidence
- Perform comprehensive assessments and apply critical thinking for symptom management across a broad range of diagnoses
- Serve as a generalist across disease processes with a specialization in end-of-life symptom management
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- Strong clinical assessment and critical thinking skills
- Experience in hospice, palliative care, or a related clinical specialty
- Ability to work a Monday–Friday schedule with occasional weekend back-up and on-call availability
Preferred Experience / Skills
- Prior experience as a hospice case manager or primary RN managing an independent patient caseload
- Experience educating patients and family caregivers in a home or community-based care setting
- Familiarity with interdisciplinary team care models in a hospice environment
