Key Responsibilities
- Provide occupational therapy services to patients in skilled nursing facilities (SNF).
- Assess patients' functional status and develop individualized treatment plans.
- Implement therapy interventions to improve mobility, independence, and overall quality of life.
- Document patient progress and maintain accurate records using NetHealth (Rehab Optima) system.
- Collaborate with interdisciplinary teams to coordinate patient care.
Required Qualifications
- Valid active California license as an Occupational Therapist.
- Minimum of 1 year of experience in a Skilled Nursing Facility (SNF).
- Knowledge of NetHealth (Rehab Optima) or similar electronic health record systems.
Preferred Experience / Skills
- Previous experience working in a SNF environment.
- Strong understanding of rehabilitation protocols and patient care standards.
