Schedule / Job Type
Contract | Full-Time | Onsite | Monday–Friday, 8:00 AM–5:00 PM with rotating on-call and weekend coverage
Key Responsibilities
- Manage an assigned caseload of hospice patients, providing direct clinical care in private homes, nursing homes, group homes, and assisted living facilities
- Lead the interdisciplinary care team for assigned patients, coordinating and guiding the overall plan of care
- Educate patients and caregivers on personal goals of care and build caregiver confidence in supporting their family members
- Perform comprehensive assessments and apply critical thinking skills across a broad range of disease processes with a focus on symptom management
- Provide occasional backup support for weekends and on-call rotations as needed
Required Qualifications
- Active Registered Nurse (RN) license in good standing
- Strong clinical assessment and critical thinking skills
- Experience in hospice, palliative care, or a related clinical specialty
- Ability to work independently in community and facility-based settings
Preferred Experience / Skills
- Experience serving as a primary or case manager RN with an assigned patient caseload
- Familiarity with interdisciplinary team collaboration in a hospice or home health environment
- Strong patient and family education skills across diverse care settings
