Contract | Full-Time | Onsite | Days (5x8 Hours, 8:00–16:00) | Monday–Friday, No Weekends, No On-Call
Key Responsibilities
- Provide discharge coordination and planning for adult inpatient medical unit patients
- Collaborate with interdisciplinary care teams to facilitate safe and timely patient transitions
- Manage a caseload ratio of approximately 20 patients
- Document assessments, care plans, and discharge summaries using Epic electronic health records
- Float between units as needed to support patient care continuity
Required Qualifications
- Active Washington State Licensed Clinical Social Worker (LCSW) license required at time of submission
- Experience in an acute care or inpatient medical setting
- Proficiency with Epic electronic health records charting system
- Ability to perform discharge planning and care coordination independently
Preferred Experience / Skills
- Prior experience in a Short Term Acute Care or Level III Trauma Center environment
- Familiarity with interdisciplinary team collaboration in a hospital setting
