Key Responsibilities

• Perform and document comprehensive patient assessments, including identification of risk factors, presenting complaints, and objective and subjective data, in accordance with RN scope of practice and applicable policies and procedures.
• Develop individualized care plans by determining desired outcomes in coordination with patients, families, and the inter-professional team, and implement nursing therapies and procedures with clinical autonomy within scope of practice.
• Conduct ongoing assessments and communicate changes in patient condition appropriately, recognizing and responding to shifts in clinical status in a timely manner.
• Facilitate admission, transfer, and discharge activities, including execution of discharge orders, coordination with Case Management, and provision of discharge education to patients and families.
• Delegate tasks appropriately to team members and monitor the accuracy and completeness of delegated patient care activities in alignment with the plan of care.
• Actively participate in unit-based quality improvement activities, including monitoring of core measures such as patient satisfaction, fall prevention, pressure ulcer prevention, and documentation auditing.

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the applicable state.
• Current Basic Life Support (BLS) certification.
• Associate Degree in Nursing (ADN) or Associate of Science in Nursing (ASN).
• Proficiency in administering medications and performing medical procedures such as wound care and IV therapy.
• Strong assessment, critical thinking, and task prioritization skills in a fast-paced clinical environment.
• Effective communication skills for collaboration with interdisciplinary teams and patient and family education.

Preferred Experience / Skills

• Bachelor of Science in Nursing (BSN).
• Experience in a cardiac stepdown or coronary care unit setting.
