Licensed Practical Nurse – Long Term Care / Post-Acute Skilled Nursing Rehab

Contract | Nights | Monday through Sunday rotating schedule | Holiday availability required | No block schedules | Weekend rotation required (minimum every other weekend)
Key Responsibilities
- Provide direct patient care on a Post-Acute Skilled Nursing Rehabilitation unit serving a patient population aged 60 and older
- Perform unit-specific procedures including phlebotomy, EKGs, patient transports, nebulizer to inhaler conversions, and starting IVs
- Operate IV pumps and document patient care using the EPIC electronic medical records and medication system
- Float to Long-Term Care and Memory Care units on an occasional or as-needed basis
- Work within a care team that includes LNA/CNA/PCT staff and unit support personnel, maintaining a nurse-to-patient ratio of 1:14
Required Qualifications
- Active LPN license in good standing
- Minimum of 1 year of clinical experience in a Long-Term Care or Post-Acute Skilled Nursing setting
- EPIC EMR experience within the past 2 years — required and non-negotiable
- Ability to work all holidays and a rotating Monday through Sunday schedule with no guaranteed days off unless specified in contract
- Reliable personal or rental transportation required; public transportation is not accessible in this area — all-season or snow tires strongly recommended November through April
- Complete work history documentation required at time of submission, including bed count, EMR system, and facility type for all positions
Preferred Experience / Skills
- IV Certification for LPNs
- Dementia capable care certification
- Prior travel nursing experience
