Key Responsibilities

• Provide skilled physical therapy assistant services to patients in a home health setting across the East Portland area and surrounding communities
• Visit up to 6 patients per day, meeting a standard productivity expectation of 30 productivity points per shift
• Complete clinical documentation using EPIC Remote Client for Home Health (laptop) and Rover mobile app, including OASIS assessments
• Manage daily patient visits independently, utilizing personal transportation to travel between patient homes with an occasional maximum radius of approximately 60 miles per day
• Collaborate with the broader care team to deliver coordinated, patient-centered rehabilitation services

Required Qualifications

• Active or pending Physical Therapy Assistant (PTA) license
• Minimum of 1 year of skilled home health experience
• Current Basic Cardiac Life Support (BCLS) certification
• Reliable personal transportation required
• Ability to work a 5x8-hour day shift schedule (no 4x10 or 3x12 availability)

Preferred Experience / Skills

• Familiarity with EPIC, Rover, and/or OASIS documentation platforms
• Experience managing an independent caseload in a home health or community-based setting
• Comfort navigating varying patient locations across a broad geographic service area
