Key Responsibilities

• Provide skilled physical therapy services to home health patients across a defined northern territory including surrounding communities
• Conduct patient visits in a home health setting, seeing up to 6 patients per day with a standard productivity expectation of 30 points per day
• Complete clinical documentation using EPIC Remote Client and Rover phone application; OASIS documentation is required
• Manage daily scheduling and travel efficiently, with an occasional maximum driving radius of approximately 60 miles and up to 90 minutes of total drive time per day
• Maintain professional business casual attire during all patient visits; scrubs are permitted
• Provide own reliable transportation for home visits throughout the service territory

Required Qualifications

• Active Physical Therapist license; compact license must be in hand at time of submission
• BCLS certification required
• Minimum of 1 year of skilled home health experience required
• OASIS competency required
• COVID-19 primary vaccination series plus one booster required
• Must have own reliable personal transportation
• Locals are accepted; candidates within a 50–75 mile radius of the facility are considered local and are welcome to apply

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting
• Experience managing a high-volume caseload with strong time and route management skills
• Comfort working independently across a multi-community territory with minimal supervision
