Contract | Days | Full-Time | 5x8-Hour Shifts | Monday–Friday | 8:30 AM – 5:00 PM | Onsite

Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs
• Float within scope of practice as needed to meet operational demands; mileage reimbursed
• Maintain accurate and timely documentation of patient care and progress
• Adhere to facility attendance and compliance policies throughout the contract period

Required Qualifications

• Active or pending Physical Therapist state license (must be obtained prior to start date)
• Minimum of 1 year of physical therapy experience (2 years preferred; considered on a case-by-case basis)
• Home health PT experience required
• Current BLS certification required
• Must reside more than 50 miles from the facility (local candidates not accepted)
• All requested time off must be submitted at the time of initial placement; late RTO requests will not be considered

Preferred Experience / Skills

• Experience with EPIC electronic health records system
• 2 or more years of physical therapy experience in a home health environment
