RN Case Manager – Breast & Cervical Care Coordinator

Key Responsibilities

  • Provide comprehensive care management for breast and cervical cancer prevention, screening, and diagnostic services, collaborating closely with providers, radiology, and the broader healthcare team
  • Perform comprehensive patient assessments and develop, implement, evaluate, and revise individualized plans of care based on patient needs and clinical circumstances
  • Administer medications and treatments safely and accurately per provider orders; recognize and respond to changes in patient condition with appropriate interventions or escalation
  • Facilitate referrals, follow-up care, and continuity of services; advocate for patients navigating the healthcare system
  • Document assessments, interventions, and outcomes accurately and timely in the electronic health record
  • Participate in quality and performance improvement activities; maintain required competencies and report adverse events or unsafe conditions per policy

Required Qualifications

  • Associate's degree in Nursing from an accredited program
  • Current, unrestricted Registered Nurse license in the State of Alaska
  • Basic Life Support (BLS) certification – American Heart Association – required at time of hire
  • Case Management Certification (CCM) required within 2 years of hire
  • Clinical competency validation required within 3 months of hire and every 3 years thereafter
  • High-risk competency validation required within 3 months of hire and annually thereafter

Preferred Experience / Skills

  • Prior experience in oncology, women's health, or a related specialty area
  • Experience with care coordination or case management in a clinical setting
  • Proficiency with electronic health record systems
  • Demonstrated ability to deliver culturally responsive, patient-centered care
