Key Responsibilities

• Perform assessments and reassessments per policy and procedure or as warranted by patient condition, demonstrating the ability to make sound independent clinical decisions
• Initiate, prioritize, and implement individualized plans of care; collaborate with patients, families, and interdisciplinary teams, revising plans as indicated
• Monitor and document patient responses to clinical interventions
• Facilitate, coordinate, and evaluate patient and family education specific to care, treatment, and discharge needs
• Delegate tasks to licensed and non-licensed personnel; serve as a mentor and preceptor while promoting teamwork and quality improvement
• Participate in quality, safety, and service process improvement efforts and comply with regulatory standards within a Practice Governance framework

Required Qualifications

• Active Registered Nurse (RN) license
• Basic Life Support (BLS) certification required
• NIH Stroke Scale (NIHSS) certification required
• Nursing Diploma or Associate's Degree in Nursing (ASN)
• Minimum of two (2) years of acute care RN experience
