We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN. Key Responsibilities:

• Provide compassionate and skilled nursing care to patients and families in a hospice setting • Conduct thorough assessments of patients' physical, emotional, and social needs • Develop and implement individualized care plans, including symptom management and education • Administer medications and treatments as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions • Provide emotional support and guidance to patients and families during the grieving process • Maintain accurate and up-to-date medical records using Net Smart documentation system

Requirements:

• AHA BLS certification • Minimum of 3 years of prior community hospice experience • Excellent typing skills and ability to navigate narrative-heavy EMR systems • Strong critical thinking and problem-solving skills • Ability to work independently and as part of a team • Compassionate and empathetic approach to patient care
