LVN / LPN LTC/SNF

Licensed Vocational Nurse/Licensed Practical Nurse - Long-Term Care/Snafu

Key Responsibilities:

• Provide high-quality patient care, including assessments, treatments, and education to residents • Develop and implement individualized care plans, in collaboration with interdisciplinary teams • Administer medications, perform procedures, and conduct vital signs as needed • Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with regulations • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care • Educate patients, families, and caregivers on health promotion, disease prevention, and management • Participate in quality improvement initiatives, reporting concerns and suggestions for improvement

Requirements:

• Current LVN/LPN licensure in the state of employment • Minimum 1-2 years of experience in LTC/SNF settings, preferably in a similar role • Strong communication and interpersonal skills, with ability to work effectively with diverse populations • Basic life support (BLS) certification, with willingness to obtain advanced life support (ALS) certification • Ability to lift, transfer, and reposition patients, as needed • Flexibility to work variable shifts, including evenings, weekends, and holidays
