LVN / LPN - Long-Term Care/Snife (LTC/SNF) Position

Join Our Team as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN)

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Snife Facility. As an LVN/LPN in our LTC/SNF setting, you will play a vital role in providing high-quality patient care and supporting the well-being of our residents.

Key Responsibilities:

• Assess, plan, coordinate, and evaluate patient care to ensure optimal health outcomes • Develop, implement, and revise individualized care plans to meet the unique needs of each resident • Administer medications, treatments, and therapies as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic nursing procedures • Educate patients, families, and caregivers on proper care techniques, disease management, and healthy lifestyle choices • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care • Document patient information accurately and thoroughly in electronic medical records • Participate in quality improvement initiatives and contribute to a culture of safety and excellence

Requirements:
