Licensed Vocational Nurse/Licensed Practical Nurse - Long Term Care/Senior Living Facilities
Job Summary:
We are seeking a compassionate and skilled Licensed Vocational Nurse/Licensed Practical Nurse to join our team in Long Term Care/Senior Living facilities. As an LVN/LPN, you will provide high-quality patient care, work collaboratively with interdisciplinary teams, and contribute to the overall well-being of our residents.
Key Responsibilities:
• Assess, plan, coordinate, and evaluate patient care to meet individual needs and goals • Develop, implement, and update individualized care plans, including medication administration and treatment plans • Provide hands-on patient care, including bathing, dressing, and grooming • Monitor vital signs, perform laboratory tests, and report results to healthcare team • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate patient care • Document patient information accurately and thoroughly in electronic medical records • Maintain a safe and clean environment, adhering to infection control and regulatory standards
Requirements:
• Current licensure as an LVN/LPN in the state of [insert state] • Minimum 1 year of experience in Long Term Care/Senior Living facilities or related healthcare setting • Strong communication and interpersonal skills • Ability to work collaboratively as part of an interdisciplinary team • Basic life support certification (BLS) and ability to obtain advanced life support certification (ALS) within 6 months of hire • Ability to lift, transfer, and position patients as needed • High school diploma or equivalent required; associate's or bachelor's degree in nursing preferred
