Licensed Vocational Nurse/Licensed Practical Nurse - Long Term Care/Senior Living Facilities

Key Responsibilities:
• Provide high-quality patient care to residents in a long term care and senior living facility setting • Conduct patient assessments, develop and implement individualized care plans, and monitor progress • Administer medications, treatments, and procedures as prescribed by physicians • Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals • Document patient information accurately and thoroughly in electronic medical records • Maintain a safe and clean environment, adhering to facility policies and regulations • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Participate in quality improvement initiatives and contribute to the development of best practices
Requirements:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of employment • Minimum 1 year of experience in long term care or senior living facilities • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment and prioritize multiple tasks • Basic life support (BLS) certification • Ability to pass a background check and obtain any necessary facility-specific certifications
