We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a Long Term Care/Senior Living Facility setting. As an LVN/LPN with our organization, you will have the opportunity to provide high-quality patient care and make a positive impact on the lives of our residents.

Key Responsibilities:

• Assess, plan, coordinate, and implement nursing care for residents, including administering medications, treatments, and therapies • Develop and implement individualized care plans to meet the unique needs of each resident • Monitor and report changes in resident condition to the interdisciplinary care team • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care • Educate residents, families, and caregivers on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date resident records and reports • Participate in quality improvement initiatives to optimize resident outcomes and satisfaction

Requirements:

• Current licensure as an LVN or LPN in the state of [insert state] • [Insert number] years of experience in a Long Term Care/Senior Living Facility setting (experience with LTC/SNF accreditation a plus) • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment with multiple priorities • Basic life support (BLS) certification and willingness to obtain advanced life support certification (ACLS, PALS, etc.)
