Key Responsibilities:
• Provide high-quality, patient-centered care to residents in a long-term care setting, including assessing, planning, implementing, and evaluating care plans. • Develop and maintain positive relationships with residents, families, and healthcare teams to ensure seamless care coordination. • Administer medications, treatments, and interventions as prescribed by physicians, following established policies and procedures. • Monitor vital signs, track progress, and report any changes or concerns to healthcare teams. • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement comprehensive care plans. • Participate in quality improvement initiatives, contributing to the development of best practices and policy updates. • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines. • Provide education and support to residents and families on health promotion, disease prevention, and management of chronic conditions.
Requirements:
• Current, unrestricted licensure as a Registered Nurse in the state of [insert state]. • Minimum 2 years of experience in long-term care or senior living facilities, with a focus on nursing care and patient outcomes. • Strong knowledge of nursing principles, gerontology, and medical-surgical care. • Excellent communication, interpersonal, and problem-solving skills. • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities. • Basic life support (BLS) certification and willingness to obtain advanced life support certifications (e.g., ACLS, PALS) as required.
