Therapist - Physical Therapist Job Opportunity

Join our supportive team at Bay State PT, where patient-centered care and exceptional outcomes come first.

We are seeking a skilled and compassionate Physical Therapist to join our outpatient clinic in Chelmsford, MA. This 13-week to 26-week contract offers flexibility in scheduling, including the option to work 4 x 10-hour days or 5 x 8-hour days, with a guaranteed 40 hours per week.

About the Role:

• Provide high-quality, patient-centered physical therapy services to patients in a fast-paced outpatient clinic setting
• Utilize a supportive care model and work collaboratively with our leadership team to deliver exceptional patient outcomes
• Participate in ongoing education and professional development to stay current with best practices and industry advancements
• Work closely with interdisciplinary teams to develop and implement individualized treatment plans

Responsibilities:

• Conduct thorough patient assessments and develop personalized treatment plans • Implement evidence-based physical therapy interventions, including exercises, modalities, and education • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination • Document patient progress and outcomes in Raintree Notes EMR system • Participate in quality improvement initiatives and contribute to a culture of continuous learning and improvement

Requirements:

• At least 3 months of outpatient experience or 1 year of PT experience in other settings • Proficiency in Raintree Notes EMR system • Strong clinical skills and ability to work independently • Excellent communication and interpersonal skills • Ability to work flexible hours, including evenings (until 7:30pm) and weekends as needed
