Job Summary:
As a Physical Therapist, you will provide high-quality patient care, develop and implement individualized treatment plans, and collaborate with healthcare professionals to achieve optimal patient outcomes. We are committed to delivering exceptional patient care and are seeking a dedicated and compassionate Physical Therapist to join our team.
Key Responsibilities:
• Develop and implement individualized treatment plans for patients, including exercises, modalities, and education • Conduct patient assessments, evaluations, and progress monitoring to ensure effective treatment • Collaborate with healthcare professionals, including physicians, nurses, and occupational therapists, to achieve optimal patient outcomes • Provide education and training to patients and families on exercises, techniques, and self-management strategies • Maintain accurate and up-to-date patient records, including progress notes and treatment plans
Requirements:
• Current licensure as a Physical Therapist in the state of Massachusetts • New graduates or experienced professionals welcome • SNF experience highly preferred • Strong clinical skills and ability to work independently and collaboratively • Excellent communication and interpersonal skills • Ability to maintain accurate and up-to-date patient records
Certifications and Background Checks:
• State license verification • Resume and references attached upon submission • Pre-screening and credentialing checklist must be completed and attached prior to submission • Background checks, including: • County criminal search (7 years for all counties lived and worked) • Federal criminal search (7 years or more) • Multi-state criminal search (7 years or more) • Nationwide sex offender search (7 years or more) • HHS/OIG search • GSA search • Social Security number trace • 1-step PPD or Quantiferon or proof of positive PPD and chest X-ray • SAM search • CORI Massachusetts background check • 8 hours of dementia training
