About the Role:

We are seeking a highly skilled Physical Therapy Assistant to join our team. As a Physical Therapy Assistant, you will work closely with our physical therapists to provide comprehensive care to patients. This is an excellent opportunity for new graduates or those with prior SNF experience to gain valuable experience and expertise in the field.

Responsibilities:

• Assist physical therapists with patient assessments, treatments, and evaluations
• Develop and implement individualized treatment plans to meet patient needs
• Conduct patient education and training on exercises and techniques
• Maintain accurate and up-to-date patient records
• Collaborate with interdisciplinary teams to ensure comprehensive care

Requirements:

• Current state license as a Physical Therapy Assistant
• New graduate or prior SNF experience preferred
• Covid-19 test results with negative results 72 hours prior to start date
• Resume and license verification attached upon submission
• Three professional references attached upon submission
• Completed Credentialing Checklist (attached)

Certifications and Qualifications:

• Current state license as a Physical Therapy Assistant
• ACHA (Updated every 5 years)
• Good moral character affidavit
