Rotation: 3 on 2 off, 2 on 3 off
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a long-term care setting. This is a great opportunity for a nurse who is looking for a challenging and rewarding role with a dynamic team.
Key Responsibilities:
- Provide high-quality patient care, including administering medications, performing vital signs, and assessing patient needs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with the interdisciplinary team to coordinate patient care and achieve optimal outcomes
- Educate patients and families on health promotion, disease prevention, and management of chronic conditions
- Participate in quality improvement initiatives to enhance patient safety and satisfaction
Required Qualifications:
- Current LVN or LPN licensure in the state of [State]
- Minimum 1 year of experience in a long-term care setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Basic life support certification (BLS) required
