Our team is passionate about making a difference in the lives of our patients and their families. **Job Summary:** As a Registered Nurse - LTAC, you will be responsible for overseeing patient care using a professional practice model. Your duties will include patient assessment, recognizing health issues, creating care plans, and assessing patient responses. You will also be responsible for securing patient travel, knowing patient needs in healthcare, and helping patients and preventing issues. **Key Responsibilities:** • Conduct detailed clinical assessments and prioritize patient and family needs • Create, implement, and evaluate care plans • Adjust plans to reach desired outcomes • Building relationships, managing discomfort, and staying professional in therapeutic connections with patients and families • Create care plans reflecting values to aid in resolving issues • Educate patients, families, nurses, and community members on healthcare choices • Assess and document learning needs regularly • Work with colleagues and community to create a care plan • Delegate tasks carefully and follow up promptly • Use different strategies for problem-solving with patients, families, and staff • Recognize limitations and prioritize safety, effectiveness, and efficiency in patient care planning and delivery • Achieve goals through teamwork, recognize contributions, commit to growth • Contribute to peers' development, show community service commitment **Qualifications:** • Grad of an Accredited Program • Specialized field of study in nursing • RN license • Must have BLS certification or get it within 30 days of starting in this role **We Offer:** • Competitive benefits package • Opportunities for growth and development • Collaborative and supportive work environment **Join Our Team:** If you are a dedicated and compassionate Registered Nurse looking for a challenging and rewarding role, we encourage you to apply.
