Licensed Vocational Nurse/Licensed Practical Nurse - Long Term Care/Snife (LTC/SNF) Location: Siler City, NC We are seeking a skilled and compassionate Licensed Vocational Nurse/Licensed Practical Nurse to join our team in a Long Term Care/Snife facility. **Key Responsibilities:** • Provide high-quality patient care, including assessments, treatments, and education to residents • Develop and implement individualized care plans to meet the unique needs of each resident • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Conduct vital signs, take medical histories, and perform other diagnostic procedures as needed • Administer medications, vaccinations, and other treatments as prescribed by physicians • Educate residents, families, and staff on various health-related topics and procedures • Maintain accurate and up-to-date records of patient information and progress **Requirements:** • Current licensure as an LVN/LPN in the state of North Carolina • 1+ year of experience in long term care or a related field • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment • Basic life support certification (BLS) required **Schedule:** • Full-time, Days shift (various schedules available) We offer a competitive compensation package and opportunities for professional growth and development.
