Key Responsibilities: • Provide direct patient care under the supervision of an RN, as per facility policies and procedures • Administer medications, treatments, and therapies as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic patient assessments • Develop, implement, and revise individualized care plans to meet patient needs • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes
Required Qualifications: • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Basic Life Support (BLS) certification • Ability to lift, transfer, and position patients safely and efficiently
Preferred Experience / Skills: • Previous experience working in long-term care or skilled nursing facilities • Knowledge of medical terminology and anatomy • Ability to work effectively in a fast-paced environment and prioritize multiple tasks simultaneously
