Key Responsibilities • Provide high-quality patient care in a long-term care or skilled nursing facility setting • Conduct thorough patient assessments, develop individualized care plans, and implement evidence-based interventions • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Educate patients and families on health promotion, disease prevention, and management of chronic conditions
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Current certification in Basic Life Support (BLS) and/or Advanced Cardiovascular Life Support (ACLS) • Strong communication and interpersonal skills
Preferred Experience / Skills • Experience working in a long-term care or skilled nursing facility setting • Knowledge of gerontological nursing principles and practices • Ability to work effectively in a fast-paced environment with multiple priorities
