Key Responsibilities • Provide direct patient care to residents in long-term care facilities and skilled nursing facilities • Conduct vital signs, administer medications, and develop individualized care plans • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Educate patients and families on health promotion, disease prevention, and management of chronic conditions
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment and prioritize multiple tasks • Basic life support certification (BLS) and CPR training
Preferred Experience / Skills • Experience working in long-term care or skilled nursing facilities • Knowledge of medical terminology and coding systems (ICD-10, CPT) • Experience with electronic medical records (EMRs) and other clinical software
