Job Title: Registered Nurse - LTC/SNF
Location: Camden, ME
Schedule / Job Type: Full-time - Days
Key Responsibilities: • Provide high-quality patient care in a long-term care setting, including assessment, planning, implementation, and evaluation of individualized care plans. • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of residents. • Monitor and report changes in patient condition, including vital signs and medication administration. • Educate patients, families, and caregivers on health promotion, disease prevention, and management of chronic conditions. • Participate in quality improvement initiatives to optimize patient outcomes and facility operations.
Required Qualifications: • Current licensure as a Registered Nurse (RN) in the state of Maine. • Minimum 2 years of experience in long-term care or skilled nursing facility settings. • Strong assessment and critical thinking skills to evaluate patient needs and develop effective care plans. • Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
Preferred Experience / Skills: • Experience working with residents with complex medical conditions, such as dementia, Alzheimer's disease, or chronic obstructive pulmonary disease (COPD). • Familiarity with electronic medical records (EMRs) and other healthcare information systems. • Certification in gerontology, geriatrics, or a related field.
