Key Responsibilities • Provide high-quality nursing care to residents in a long-term care setting, including administering medications, changing wound dressings, and monitoring vital signs. • Develop and implement individualized care plans to meet the unique needs of each resident, in collaboration with the interdisciplinary care team. • Educate patients and families on proper care techniques, disease management, and prevention of complications. • Collaborate with physicians and other healthcare professionals to coordinate resident care and ensure seamless transitions.
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NH. • Minimum of 1 year of experience in long-term care or similar setting. • Basic life support (BLS) certification.
Preferred Experience / Skills • Experience working in a skilled nursing facility (SNF) or assisted living facility (ALF). • Knowledge of medical documentation and record-keeping procedures. • Strong communication and interpersonal skills.
