Schedule / Job Type Full-time, Nights
Key Responsibilities • Provide high-quality patient care in a long-term care and skilled nursing facility setting • Conduct patient assessments, develop individualized care plans, and implement interventions to achieve optimal patient outcomes • Administer medications, treatments, and therapies as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
Required Qualifications • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NH • Basic Life Support (BLS) certification • Ability to lift, transfer, and position patients safely and effectively
Preferred Experience / Skills • Experience working in long-term care and skilled nursing facilities • Knowledge of medical terminology and ability to communicate effectively with patients and families • Ability to work independently and as part of an interdisciplinary team
