Key Responsibilities:

• Provide physical therapy assistance to patients under the supervision of a licensed physical therapist • Conduct patient assessments, create treatment plans, and implement exercises and modalities as directed • Maintain accurate and detailed patient records, including progress notes and treatment plans • Collaborate with the physical therapy team to ensure comprehensive patient care

Required Qualifications:

• State License Verification as a Physical Therapy Assistant • Current Resume • References (3) • County Criminal Background Check (7 years for all counties lived and worked) • Federal Criminal Background Check (7 years or more) • Multi-State Criminal Background Check (7 years or more) • Nationwide Sex Offender Search (7 years or more) • HHS/OIG Search • GSA Search • Social Security Number Trace • PPD or Quantiferon test with proof of positive result and Chest X-Ray

Preferred Experience / Skills:

• SNF experience preferred • Covid-19 vaccination required (proof of vaccination within 72 hours of start date and results issued 48 hours prior to start date)
