Key Responsibilities:

• Provide occupational therapy services to patients in a skilled nursing facility setting.
• Develop and implement individualized treatment plans to meet patient needs.
• Assess patient progress and adjust therapy plans accordingly.
• Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care.
• Document patient treatments and progress accurately in electronic health records.

Required Qualifications:

• Active California occupational therapy license.
• Minimum of one year of experience in a Skilled Nursing Facility (SNF).
• Knowledge of NetHealth (Rehab Optima) or similar electronic documentation systems.
• Strong interpersonal and communication skills.
• Ability to work effectively in a team environment.

Preferred Experience / Skills:

• Prior experience working with diverse patient populations in SNF settings.
• CPR/BLS certification.
