LVN / LPN LTC/SNF
Job Title: LVN / LPN - LTC/SNF
Location: Berlin, VT
Schedule: Nights
Key Responsibilities: • Provide high-quality nursing care to residents in a long-term care and skilled nursing facility setting • Develop, implement, and evaluate individualized care plans for residents • Monitor and report changes in resident condition to the healthcare team • Administer medications and treatments as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate resident care
Required Qualifications: • Current LVN / LPN license • 3+ years of experience in LTC/SNF setting • BLS certification through AHA • Ability to work independently and as part of a team • Strong communication and problem-solving skills
Preferred Experience / Skills: • Experience with IV therapy • Familiarity with electronic medical records (EMRs) • Ability to work flexible schedules, including weekends and holidays • Experience with dementia care and supporting residents with cognitive impairments
