Key Responsibilities • Provide high-quality patient care to residents in a long-term care or skilled nursing facility setting • Assess patients' medical needs and develop individualized care plans • Administer medications, treatments, and therapies as prescribed by physicians • Document patient progress and communicate with healthcare teams to ensure coordinated care
Required Qualifications • Current licensure as an LVN/LPN in the state of VA • Basic Life Support (BLS) certification • Ability to work effectively in a fast-paced, dynamic environment
Preferred Experience / Skills • Previous experience working in LTC/SNF settings • Strong communication and interpersonal skills • Ability to adapt to changing patient needs and priorities
