LVN / LPN LTC/SNF
**Licensed Vocational Nurse/Licensed Practical Nurse - Long Term Care/Senior Living**
**Location:** Franklin, NH
**Schedule:** 2-week schedule, Sunday-Friday nights (Week 1: Sunday, Friday, Saturday; Week 2: Sunday, Tuesday, Thursday)
**Job Type:** Full-time
**Required Qualifications:**
• Current licensure as an LVN/LPN in the state of NH
• 1+ year of experience in a long term care or senior living facility
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
**Key Responsibilities:**
• Provide high-quality patient care, including administering medications, monitoring vital signs, and performing various medical procedures
• Develop and implement individualized care plans for residents
• Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions
• Maintain accurate and up-to-date patient records
• Educate patients, families, and staff on various health-related topics
