Key Responsibilities

• Provide skilled nursing care for residents in long-term care and skilled nursing facility settings.
• Assist with assessments, medication administration, and patient documentation using PCC electronic medical records.
• Collaborate with a team of nurses to ensure quality resident care, maintaining appropriate staffing ratios.
• Support residents with memory care, dementia, orthopedic needs, and general LTC diagnoses.
• Follow facility-specific protocols for lifts and patient safety during orientation and daily procedures.
• Adhere to facility policies regarding on-call, callbacks, and notice periods for shift changes or cancellations.

Required Qualifications

• Valid LVN or LPN license in the state of South Dakota.
• Prior experience in long-term care (LTC) or skilled nursing facility (SNF) environment.
• Comfortable working with residents with memory care and orthopedic needs.
• Knowledge of PCC or similar electronic health records systems.
• Ability to work the scheduled mid-shift hours reliably.

Preferred Experience / Skills

• Experience with resident care involving dementia or memory impairments.
• Familiarity with lifts and patient transfer techniques.
• Ability to work independently and within a team in a fast-paced LTC setting.
