Registered Nurse - LTC/SNF

Key Responsibilities • Provide high-quality nursing care to residents in a long-term care and skilled nursing facility setting • Develop, implement, and evaluate individualized care plans to meet the unique needs of each resident • Collaborate with interdisciplinary teams to coordinate resident care and services • Monitor and report changes in resident condition, including medical, emotional, and functional status
Required Qualifications • Current licensure as a Registered Nurse (RN) in the state of VT • Minimum of 2 years' experience in long-term care or skilled nursing facilities • Valid BLS certification through AHA • Ability to work independently and as part of an interdisciplinary team
Preferred Experience / Skills • Previous experience working in LTC/SNF settings • Familiarity with IV therapy and medication administration • Strong communication and interpersonal skills
