Key Responsibilities: • Provide occupational therapy services to adult patients in their homes, focusing on activities of daily living, mobility, and adaptive equipment use • Conduct assessments, develop treatment plans, and implement interventions to improve patients' functional abilities and quality of life • Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive patient outcomes
Required Qualifications: • WA license • NBCOT certification • BCLS certification • 1 year of recent skilled hospice experience within the last 3 years (inpatient experience not accepted)
Preferred Experience / Skills: • Experience working with adult patients with diverse needs and diagnoses • Familiarity with HomeCare HomeBase documentation tools • Ability to work independently and as part of a team to achieve patient outcomes
