Job Title: LVN / LPN - LTC/SNF
Location: Rutland, VT
Schedule: Nights
Key Responsibilities:
• Provide high-quality patient care to residents in a long-term care and skilled nursing facility setting • Assess, plan, implement, and evaluate patient care plans to meet individual needs • Administer medications, treatments, and therapies as prescribed by physicians • Collaborate with interdisciplinary teams to develop and implement care plans
Required Qualifications:
• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Current certification in CPR and basic life support • Ability to lift, transfer, and position patients as needed
Preferred Experience / Skills:
• Previous experience working in long-term care or skilled nursing facilities • Knowledge of medical terminology and ability to communicate effectively with patients and families • Ability to work independently and as part of a team in a fast-paced environment
