Job Title: LVN / LPN - LTC/SNF

Location: Malone, NY

Schedule / Job Type: Mids

Key Responsibilities:

• Provide skilled nursing care to long-term care and skilled nursing facility residents.
• Administer medications and treatments as per physician’s orders.
• Monitor patient conditions and report any significant changes.
• Cover CNA duties as needed, ensuring comprehensive patient care.
• Maintain accurate documentation of patient care activities and compliance with facility policies.
• Follow all safety and confidentiality protocols.

Required Qualifications:

• Valid NY State LVN/LPN license.
• AHA BLS certification (please attach proof).
• Minimum of 1 year LTC or SNF experience.
• Successful completion of a 7-year background check, including FACIS 3 and NY State check prior to start.
• Current licenses and clear background checks on file at time of submission.
• Must be able to start within the next 2 weeks.

Preferred Experience / Skills:

• Experience with electronic medical records (EMR).
• Ability to work independently and as part of a team.
• Reliable transportation, personal or rental vehicle, with all-season or snow tires during November-April.
