Key Responsibilities

• Assess patient needs and develop personalized treatment plans to improve mobility and function.
• Provide evidence-based physical therapy interventions in a Skilled Nursing Facility setting.
• Document treatment progress and communicate effectively with healthcare teams.
• Ensure compliance with all regulatory and safety standards.
• Educate patients and their families on therapeutic exercises and injury prevention.

Required Qualifications

• Valid state licensure as a Physical Therapist.
• Current license verification.
• Ability to pass a background check, including criminal, sex offender, and federal searches.
• Proof of COVID-19 testing within 72 hours of start date, with results issued 48 hours prior.
• Must complete and attach prescreening, resume, and license verification documents with submission.

Preferred Experience / Skills

• Skilled Nursing Facility (SNF) experience preferred.
• New graduates are encouraged to apply.
