LVN / LPN LTC/SNF

**Licensed Vocational Nurse/Licensed Practical Nurse - Long Term Care/Snife Care** **Location:** Franklin, NH **Schedule:** 2-week schedule, Sunday to Saturday, with varying shifts per week **Job Type:** Full-time **Key Responsibilities:** • Provide high-quality patient care in a long-term care and skilled nursing facility setting • Assess, develop, and implement individualized care plans for patients • Administer medications, treatments, and therapies as prescribed • Monitor patient vital signs and report any changes or concerns to the healthcare team • Educate patients and their families on health promotion, disease prevention, and management of chronic conditions • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes **Required Qualifications:** • Current licensure as an LVN/LPN in the state of NH • 1+ year of experience in long-term care or skilled nursing facility setting • Strong assessment and critical thinking skills • Ability to work effectively in a fast-paced environment with multiple priorities • Excellent communication and interpersonal skills **We are an equal opportunities employer and welcome applications from diverse candidates.**
