Job Title

Key Responsibilities
• Provide skilled physical therapy assistance to patients in a home health setting.
• Conduct 1:1 patient visits, managing a caseload of up to 4-5 patients per day.
• Document patient progress and care plans accurately using provided systems.
• Collaborate with team members to ensure efficient and quality patient care.
• Travel within the assigned territory, with minimal drive time to maximize productivity.

Required Qualifications
• Minimum of 1 year of recent skilled PTA experience within the last 3 years (in outpatient or home health; inpatient experience not accepted).
• Current BLS certification.
• Valid Washington state license or compact license.
• Own reliable transportation for travel within territory.

Preferred Experience / Skills
• Prior experience working in a home health or outpatient setting preferred.
• Ability to work independently and communicate effectively with team and patients.
