Key Responsibilities
- Provide speech-language pathology services to home health patients in Medford and surrounding areas.
- Evaluate and develop treatment plans tailored to individual patient needs.
- Perform therapy sessions, ensuring proper documentation within electronic health record systems such as EPIC/Oasis.
- Travel within a designated radius to visit a maximum of six patients per day, adhering to productivity standards.
- Maintain compliance with all facility and regulatory requirements, including BCLS certification.
Required Qualifications
- Minimum of 1 year of skilled home health experience as a Speech Language Pathologist.
- Current license to practice as a Speech-Language Pathologist, pending license accepted.
- BCLS certification required.
- Valid driver’s license and reliable transportation to travel within assigned regions.
Preferred Experience / Skills
- Experience working with home health populations and conducting therapy in a home setting.
- Familiarity with electronic charting systems such as EPIC and Oasis.
